DepEd Enhanced Basic Education Enrollment Form
The Department of Education's online and modular learning is almost totally over come August 22, 2022 when most public school students will go back to school face-to-face. The department, thus, releases guidelines on the School Calendar and Activities for School Year 2022-2023 in accordance with its commitment to the resumption of 5 days of in-person classes.
Signed by the concurrent Vice President of the Philippines and Secretary of the Department of Education Sara Z. Duterte, DepEd Order No. 034, series of 2022, sets the beginning of classes on August 22 which is mostly face-to-face and will end on July 7, 2023. There will be 203 school days or as may be determined by further issuance/s in case of changes in the school calendar due to unforeseen circumstances.
DepEd only prescribed the options of 5 days person in-person classes, blended learning modality, and full distance learning until October 31, 2022.
Starting November 2, 2022, all public and private schools shall have transitioned to 5 days of in-person classes. No school shall be allowed to implement purely distance learning or blended learning except for those implementing Alternative Delivery Modes as provided in DO 21, s. 2019 (Policy Guidelines on the K to 12 Basic Education Program) and DO 01, s. 2022 (Revised Policy Guidelines on Homeschooling Program).

All you need to do is to download the form from this site and fill it out with the necessary information being asked.
Enrollment has been going on since July 25, 2022 and it will end on August 22 of the same year.
Here are some words take from (Enclosure No. I to DepEd Order No. 034 s. 2022) IMPLEMENTING GUIDELINES ON THE SCHOOL CALENDAR AND ACTVITIES FOR THE SCHOOL YEAR 2O22-2O23.
The School Year 2022-2023 shall formally start on Monday, August 22, 2022, and end on Friday, July 7, 2O23. There shall be a total of 2O3 days or as may be determined by further issuance/s in case of changes in the school calendar due to unforeseen circumstances.
No inspections, tools, or any additional requirements to re-open schools and to implement 5 days in-person classes shall be required, except for the compliance with the usual pre-pandemic regulatory permits and licenses, as required by law or ordinances. Read more here.
