Quarterly Report on Assessment (QRA) for Grades 1 to 6
The Department of Education in the Philippines requires all public elementary schools to submit quarterly assessment reports for grades 1 to 6. This report is used to track the progress of students in their learning areas.

The Form 1 Class Profile (KS1) is the official form used for this report. It includes sections for the following information:
- School ID, school year, and section
- School name
- Learner’s name
- Average score for each learning area
- Number of students who got each grade (Outstanding 90-100, Very Satisfactory 85-89, Satisfactory 80-84, Fairly Satisfactory 75-79) for each learning area
- Prepared by (adviser)
- Reviewed and validated by (school testing coordinator)
- Noted by (school head)

- Fill out the form with the required information.
- Submit the completed form to the school testing coordinator.
