How to Apply for a PWD ID & Booklet

Persons with Disabilities (PWDs) in the Philippines are entitled to certain benefits and privileges under Republic Act No. 10754, also known as the Act Expanding the Benefits and Privileges of PWDs. This law, sponsored by Senator Sonny Angara, was signed into law 8 years ago and provides a 12% VAT exemption on top of a 20% discount for medicines and essentials, similar to the benefits enjoyed by senior citizens.

Where to Apply

Applications for a PWD ID and booklet can be made at the following locations:

  • Office of the Mayor
  • Barangay office
  • National Council on Disability Affairs (NCDA)
  • Department of Social Welfare and Development (DSWD) Offices
  • Participating organizations with memorandum of agreements with the Department of Health

What to Bring

Applicants are required to bring the following documents:

  • A clinical abstract duly signed by any licensed private or government clinic or hospital-based physician. The clinical abstract will be used as a basis to match the actual health condition of the applicant.
  • Two (2) copies of the applicant’s 1x1 picture with thumbmark and signature at the back.

Benefits for Cardholders

PWD ID cardholders are entitled to discounts and VAT exemptions for the following:

  • Medical and dental services
  • Land transport, domestic air, and sea travel
  • Restaurants and recreation centers
  • Concerts, circuses, carnivals, and other places of culture, leisure, and amusement
  • Funeral and burial services
