The job fair will be held in various locations across the country including:
- NCR and Luzon: Dr. Jose Fabella Memorial Hospital; SM San Lazaro (June 18)
- Cagayan Valley and Central Luzon: SM City Tuguegarao (June 20 only); SM City Telabastagan (June 18 only)
- Bicol Region: SM City Legazpi
- Visayas: Western Visayas Medical Center; SM City Cebu (June 18 only)
- Mindanao: SM Mindpro (June 18 only); Ayala Malls Abreeza; SM Butuan (June 19 only)
Application Requirement Checklist
Interested applicants are required to prepare the following:
- Letter of Intent addressed to the Head of Human Resources of the Health Facility
- Updated Personal Data Sheet (CS Form 212, Revised 2017, downloadable at https://www.csc.gov.ph/), with attached Work Experience Sheet
- Certified True Copy of Diploma, Transcript of Records or Certificate of Units earned (for undergraduate), or Form 137 (for HS/Elementary)
- CSC Eligibility and/or PRC License and Board Rating
- Copy of most recent Performance Evaluation (if applicable)
- Copy of Certificates of Trainings/Seminars relevant to the position
- Certificate/s of Employment
- Proof of Membership in Society or Professional Organizations (if applicable)
- Proof of Awards received (Professional or Civic related)
