3. Brief Facts of the Case

Summarize the nature of the incident and key facts discovered so far.

Example:
Initial investigation revealed that two unidentified suspects, armed with bladed weapons, entered the residence of Mr. Juan Dela Cruz and carted away valuables worth approximately PHP 150,000. There were no reported injuries.


4. Other Facts

Include any additional relevant information or developments, such as witness accounts, CCTV footage, or forensic evidence.

Example:
CCTV footage from a nearby establishment captured the suspects fleeing the scene aboard a black motorcycle without license plates.


5. Requested/Recommended Actions from Hqs.

Clearly specify what actions, approvals, or resources are needed from the higher command or headquarters.

Example:
Requesting authority to coordinate with NCRPO’s Criminal Investigation and Detection Group (CIDG) for further technical assistance.


6. Disposition/Action Taken

Indicate steps already taken by your unit.

Example:

7. Final Report Will Follow

End the memo by noting that a full report will be submitted once the case progresses or is resolved.

Example:
A Final Report will be submitted upon conclusion of the investigation.
